C3-Cloud

Multi-morbid conditions call for care to be restructured around the patient and for all stakeholders involved to be engaged. Recognising this demand, the EU-funded C3-Cloud aimed to revolutionise integrated care by harnessing the power of information and communications technology (ICT). The core objective was to establish an ICT infrastructure that enabled seamless and collaborative coordination of patient-centric care activities, addressing the complex challenges inherent in multi-morbid conditions. The project brought together a diverse team of healthcare professionals, patients, and informal caregivers to optimise disease management and training guidelines. Ultimately, the project endeavoured to ensure continuous, high-quality care that was tailored to the unique needs and circumstances of individuals living with multiple health conditions.
